Administration of Medications (both prescription and over-the-counter)

Any parent wishing the school to administer their child medication - both prescription and over-the-counter (OTC) during the school day will need to complete this School Medication Authorization Form which requires both a doctors and parents completion and signature.  Administration of Medication Policy

 

Physicals with Immunizations:

Students entering Kindergarten, 6th, and 9th grades are required to have a physical with immunizations and lead screening.

*Please be aware of the new Tdap immunization requirement for all incoming 6th graders only.

*Also, be sure to complete the parent box at the top of the physical form, sign and date before turning into the school office. Your child's completed physical form can be sent in the same envelope with tuition/school fees.

All physical requirements MUST be completed and sent to the school office by the first day of school.  Appointment dates are not acceptable. Click here for the flyer from the Peoria Health Department.

IESA Sports Physicals

ALL students in 7th and 8th grades participating in sports (not PPSA soccer) and cheerleading MUST turn in a sports physical to the school office prior to the season start date.

Fall Semester sports start dates:

1) Softball - Monday, July 25th

2) Cross Country/Baseball - Monday, August 1st

3) Golf - Monday, August 15th

4) Girls Basketball - Monday, August 29th

5) Boys Basketball/Cheerleading - Monday, October 17th

6) Girls Volleyball - Monday, November 28th

Spring Semester sport start date:

1) Track - Monday, February 27, 2023

This sports physical requirement does not include fall parochial soccer since that is a Parish sport.

Eye Exams:

Students entering Kindergarten must present proof of being examined by a qualified eye doctor – such as optometrists and ophthalmologists. Your child's completed eye exam can be sent in the same envelope with tuition and school fees. See form below.

Dental Exams:

Students entering Kindergarten, 2nd grade, and 6th grade or are new to the school must present proof of being examined by a dentist. Your child's completed dental form can be sent in the same envelope with tuition/school fees. Please send dental forms to the school office as soon as completed. See form below.
