FACTS School Information System (SIS)  gives you, the busy parent, an opportunity to get more involved in your child's academic progress and future success – all via the Internet!

FACTS SIS' Family Portal is a private and secure parents' portal that will allow parents to view academic information specific to their children, while protecting their children's information from others. You can see your child's grades, attendance, homework and conduct, as well as other useful school information. You can also communicate with teachers and other school staff online whenever necessary. All you need is an Internet-capable computer.

Webforms to be Completed Online:

Log into Family Portal (new families see attached Family Portal Log-In instructions), under School Information tab, click on WebForms tab.

1) When completing WebForms, please keep in mind that ALL PHONE NUMBERS MUST HAVE AREA CODES and ALL DATES MUST BE: mm/dd/yyyy.

2) There is a SAVE button at the top and bottom of the form that you must click on one of them when finished updating/completing your form to "SUBMIT" to the school – (SAVE in Family Portal = SUBMIT to school). When you have clicked on SAVE, it is necessary to either click on the School Information, WebForms tab again or click on the back arrow to update/complete a different form (A, B, or C below).

A) Signature Form – 1 per family

B) Student Field Trip Medical Form – 1 per student Existing student's information is still completed on the form. Please just update any information if necessary and then the DATE MUST be changed before saving or your form will not be received.

*Families who completed online re-enrollment in the spring do not need to complete the Family Demographic forms below.

*New families/K students – please complete the Family Demographic forms below.

C) Family Demographic Forms –Student Demographic, Student Medical, Custodial Parent, and Emergency Contacts (need 2 other contacts besides parents)
